GH Pulse Timing and Peptide Injections: The Research

Growth hormone is released in pulses rather than continuously, and this article reviews the physiology and limits of peptide-related timing research.

The growth hormone axis does not operate continuously. It works in pulses — brief, high-amplitude bursts of GH secretion separated by periods of low or undetectable output. This pulsatile pattern is not incidental to how GH functions. It is fundamental to it. And it is why the timing of GH-axis peptide injections matters as much as the dose.

The Biology of the GH Pulse

GH secretion from the pituitary gland follows a circadian rhythm, with the largest pulse occurring 60 to 90 minutes after sleep onset — during the first phase of deep sleep. This pulse is driven by the interplay of growth-hormone-releasing hormone (GHRH) and somatostatin, with additional input from ghrelin-receptor signalling. GH-axis peptides — including Ipamorelin, CJC-1295, and Sermorelin — work by amplifying or extending this natural pulse rather than replacing it.

Why Timing the Injection Matters

Research on GH physiology suggests that administering a GH-axis peptide at bedtime places the compound in circulation when GHRH drive is highest and somatostatin tone is lowest, the window associated with the natural nocturnal pulse. Morning administration, when somatostatin is elevated and the GH axis is relatively suppressed, has been associated with a blunted response. How this applies to any individual is a decision for the prescribing physician.

  • Research links the largest natural GH pulse to the first hours of sleep
  • Studies associate a fasted state with higher GH pulse amplitude
  • High insulin after a recent meal has been shown to blunt the GH response
  • Timing and dose for any protocol are set by the prescribing physician
