Bremelanotide — known by its research designation PT-141 — is a synthetic analogue of the melanocortin-stimulating hormone alpha-MSH. Unlike most compounds used for sexual dysfunction, it does not act on the vascular system. It acts centrally, through the brain's melanocortin receptors, which is why research has focused on the neurological and motivational components of sexual arousal rather than the haemodynamic ones.
Mechanism of Action
PT-141 is an agonist of the melanocortin 4 receptor (MC4R) in the central nervous system. MC4R is expressed in brain regions involved in sexual motivation, including the hypothalamus. Activation of this receptor pathway has been studied in relation to sexual desire and arousal, through the neurobiological signals involved in libido. Because the mechanism is central, research interest has focused on cases where low desire is not primarily vascular.
Clinical Evidence and Applications
Bremelanotide is registered in the US, under the name Vyleesi, for a specific indication: hypoactive sexual desire disorder in premenopausal women. Outside that indication, research in men and women is more limited, and whether it is relevant to any individual is a decision for the prescribing physician after a full assessment, including a hormone panel and health history review.
