Tesamorelin: A GHRH Analogue and What the Research Shows

Tesamorelin is a synthetic analogue of growth-hormone-releasing hormone, examined in clinical research for a specific disease-related indication.

By UAE Peptide Clinic Research Desk

Tesamorelin is a 44-amino-acid synthetic analogue of growth-hormone-releasing hormone (GHRH). Its design includes a specific N-terminal modification (a trans-3-hexenoic acid group) that confers resistance to the enzyme dipeptidyl peptidase IV (DPP-IV), substantially extending its half-life compared to native GHRH. It is a GHRH analogue registered in the US for a specific indication: HIV-associated lipodystrophy, with reduction of excess visceral adipose tissue in that population.

Mechanism of Action

Tesamorelin binds to the GHRH receptor on pituitary somatotrophs and stimulates the release of endogenous growth hormone. The downstream effect is an elevation of circulating IGF-1, which in trials was associated with reductions in visceral (intra-abdominal) fat. Unlike growth hormone itself, Tesamorelin acts through the body's own feedback mechanisms, maintaining a more physiological response pattern than direct GH administration.

Clinical Applications and Evidence

Phase III trials in adults with HIV-associated lipodystrophy reported visceral fat reductions of roughly 15 to 20 per cent over 26 weeks, with modest changes in lipid profiles. Other research has examined its role in non-alcoholic fatty liver disease, age-related metabolic decline, and cognitive function in older adults. Whether it is appropriate for any individual is a decision for the prescribing physician.

  • Administered by subcutaneous injection in trials
  • Registered in the US for HIV-associated lipodystrophy
  • Visceral fat reductions of 15-20% reported in Phase III trials in that population
  • IGF-1 monitoring required throughout protocol
