Ipamorelin is a synthetic pentapeptide and selective agonist of the growth hormone secretagogue receptor (GHS-R), the ghrelin receptor. Among GH-stimulating peptides, it is notable for what it does not do: in studies it has not meaningfully elevated cortisol, prolactin, or appetite at the doses examined. This selectivity profile is why it is widely discussed in physician-supervised longevity medicine.
Mechanism of Action
Ipamorelin binds GHS-R in the pituitary and hypothalamus, stimulating GH release through a pathway independent of GHRH signalling. When studied alongside a GHRH analogue (such as CJC-1295 or Sermorelin), the two compounds provide complementary inputs into the same system: GHRH acts on pulse amplitude, while Ipamorelin adds a distinct ghrelin-mediated stimulus. Research suggests the combined effect on GH release may exceed that of either compound alone.
What Research Has Examined
Research on GH pulsatility has examined body composition (adiposity and lean mass), sleep architecture, recovery from physical exertion, and skin. Because Ipamorelin has not been associated in studies with the cortisol or prolactin effects of earlier GHRPs, it has attracted particular research interest.
